Sauk Valley Senior Living, a 55-bed skilled care facility in Rock Falls, was cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the first quarter of 2019.
The report lists the nursing homes in the state with A violations, which might have caused death or serious injury to a resident.
Sauk Valley Senior Living was cited with two A violations for incidents in November and December of last year, and fined a total of $50,000.
